ID Cards

ID cards are only available digitally through HealthWallet. If you would like a physical medical card, detnal card, or vision card, please contact your benefit plan administrator. 

For dependent ID cards, please login using the dependent SSN and date of birth, or their member ID and Date of birth.

TIP - Digital cards can be emailed directly from your HealthWallet profile in the App.
